Do you give a mid-course survey to your students? You can use it to find out how things are going, where there are problems, and whether the course is succeeding in the ways that you want. It can provide you with this information, and give the students an opportunity to reflect on the course as a whole.
A faculty member at another school gives a set of questions and then posts the responses on-line for everyone to see. An example is at http://people.stu.ca/~hunt/32360708/evals/ so you can see the types of questions and answers he received.
You can do this with paper/pencil, or use Blackboard's survey system to collect the answers anonymously. If you would like assistance with this, let me know.
